  • Introduction: Caries are local destruction of teeth by bacterial byproducts of carbohydrate
fermentation. The formation of caries begins with the activity of the biofilm that covers healthy
teeth. Streptococcus mutans acts as the dominant bacteria in the biofilm, forms multispecies
biofilm on the tooth surface, grows and survives in the biofilm. Colony addition and acid
formation by S. mutans can lower the pH of the oral cavity and cause tooth demineralization
as an early sign of caries formation. This literature review aims to determine the ability of
Streptococcus mutans to survive in biofilms and during dental caries formation in more depth
from the articles available in the selected database. Methods: The articles searched on
PubMed, ScienceDirect, Cochrane, and Google scholar databases with keywords
“Streptococcus mutans and caries or dental caries and survival ability or survivability and
survival factor” in the last five years. The articles then selected based on the inclusion and
exclusion criteria using PRISMA Scr. Results: Ten articles were suitable for review. It
presented varies data from location, purpose, method, and samples taken in the study. The
results showed that S. mutans were able to survive in biofilms and caries formation due to its
ability to activate enzymes and virulence factors of S. mutans supported by the environmental
conditions of the oral cavity. Conclusion: The ability of Streptococcus mutans to survive in
biofilms and during caries formation are: aciduric; acidogenic; quorum sensing; ability to
form GTFs, GBPs, ATPase, CSP, eDNA; and the ability to produce mutations and autolysins.
Keywords: biofilm, caries, Streptococcus mutans, survival ability, survival factor.
