  • Basic fundamental concepts of orthodontic is undergoing a paradigm shift and greater attention is paid to soft tissue aspects. The most common single characteristic of Class II malocclusion is mandible retrognathism. The Twin Block appliance is often used to correct retrognathic mandible. The purpose of this study is to determine the changes in soft tissue facial profile of Class II skeletal malocclusion patient with retrognathic mandible after Twin Block treatment.
    The type of research is retrospective descriptive study. The sampling method used is total sampling. A total of six patients treated from year 1995 to 2015 in the Orthodontic Department of Rumah Sakit Gigi dan Mulut fulfilled the inclusion criteria and soft tissue analyses are carried out on the samples collected.
    The results showed that the Soft Tissue Profile angle and Holdaway�s Soft Tissue angle increases after Twin Block treatment. However the difference that occurred is not significant (P > 0.05). Whereas, Holdaway�s H angle decreases and Merrifield�s Z angle increases after Twin Block treatment and the difference is significant (P < 0.05).
    The conclusion of this study is the Twin Block appliance can stimulate the growth of the soft tissue chin in a downward and forward direction hence causes significant increase of Z angle and significant decrease of H angle after Twin Block treatment. A significant decrease of H angle indicates a reduction in facial convexity and improvement of facial profile after Twin Block treatment.
