  • Typhoid fever remains a major public health problem in Indonesia due to its persistent transmission and substantial disease burden. However, comprehensive national-level epidemiological evidence describing rate of incidence (IR), prevalence (PR), claim-based mortality rate (CBMR), and spatial distribution patterns remains limited. This study aimed to analyze the epidemiological burden and spatial distribution of typhoid fever in Indonesia using Indonesian National Health Insurance (NHI [JKN]) real-world claims data from 2019 to 2024. A retrospective repeated cross-sectional study was conducted using Indonesian NHI claims data, with typhoid fever cases identified through ICD-10 code A01. IR, PR and CBMR were calculated per 100,000 population with 95% confidence intervals, while spatial distribution was assessed using choropleth maps and heatmaps. A total of 106,030 typhoid fever claims were identified from 17,865,889 healthcare claims records during the study period. The IR rate declined from 409.65 per 100,000 population in 2019 to 148.74 in 2021 before increasing to 289.52 in 2024. A similar trend was observed for PR, which decreased from 409.65 in 2019 to 174.54 in 2021 and subsequently increased to 341.13 in 2024. The CBMR remained consistently low, ranging from 0.5126 per 100,000 population in 2019 to 0.0007 in 2024. Spatial analysis revealed considerable geographical heterogeneity, with several provinces consistently exhibiting higher disease burdens than others, indicating potential persistent endemic areas. These findings demonstrate that typhoid fever remains an important public health concern in Indonesia, particularly following the post-pandemic increase in IR and PR. Strengthening surveillance systems, improving water, sanitation, and hygiene (WASH) programs, and expanding typhoid conjugate vaccine (TCV) implementation are essential to reduce the future burden of disease.
