  • Hemophilia is a prevalent severe inherited hemorrhagic disease caused by factor protein deficiency, characterized by prolonged bleeding after vascular injury. Hemophilia affects millions of people worldwide, including in Indonesia. The high cost of disease management places a substantial economic burden on healthcare systems and patients. This retrospective repeated cross-sectional analysis uses BPJS Kesehatan health insurance claims data from 2019 to 2023 and STATA ver. 18 software to assess the entire economic burden and cost drivers of hemophilia in Indonesia. The survey-weighted Generalized Linear Model (GLM) with gamma and log-link function used to discover independent drivers of healthcare costs. With a total of 3,507 weighted hemophilia cases and 136 individuals included, total healthcare spending rose from IDR 471 billion in 2019 to IDR 848 billion in 2023, demonstrating a growing economic burden. The geographic region was the most consistent independent cost driver. Insurance and hospital type had no substantial impact on cost variation in any of the study years. These findings provide nationally representative evidence to support resource allocation, tariff policy, and geographically targeted hemophilia management measures under Indonesia's universal health care framework.
